We would like to thank you for considering Dayton Islamic School for your child's education.  Here are few things to know before enrolling.

 

Before a student may be enrolled at DIS, parents must provide proof of immunization or certification of exemption from immunization requirements. Students applying to any grades may be required to take an admissions test.

Any applicant interested in registering at DIS may contact the school to recieve an application and to inquire about open registration times.  A $25 non-refundable application fee applies to all new applicants.

Any application for registration will be submitted to the DIS office along with the required fee. The administration will review all materials and documents to determine the acceptance of each applicant.

An application for admissions will not be complete unless accompanied by the following forms:

• A copy of the student's birth certificate or passport verifying age.

• Transcripts and School Records for each year of attendance at another school.

• Proof of immunizations.

• Copy of social security card.

• Proof of custody if not living with both natural parents.

• Complete address of previous school.
